Oregon, Jerusalem Township, and Toledo firefighters responded to a residential structure fire on Ansonia Street near Wheeling Street on Sunday, July 26, 2026.

One victim was found deceased inside the residence. The victim’s identity has not been released. The State Fire Marshal’s Office is investigating the cause of the fire. No firefighters were injured.

Emergency scenes don’t always end when the apparatus leave.

More than a week after five members of a Toledo family died following a suspected carbon monoxide incident inside a disabled vehicle, a roadside memorial continues to grow along Phillips Avenue. Candles spelling out each of their names, along with flowers and stuffed animals, now mark the place where the family is being remembered—a quiet reminder of a tragedy that continues to weigh heavily on the Toledo community.

Toledo Fire & Rescue Department responded to a residential structure fire in the 3000 block of Chase Street on Sunday, July 19, 2026. Multiple callers reported heavy fire from a house with concerns it was spreading to a neighboring residence.

Firefighters arrived to find heavy fire conditions and initiated an interior attack. The bulk of the fire was extinguished before the incident commander ordered all crews out of the structure due to concerns over a sagging roof and deteriorating structural stability. Truck 5’s aerial platform was then used to ventilate the roof, access remaining hotspots, and extinguish fire in the attic.

No injuries were reported. The cause of the fire remains under investigation.

TFRD responded to a motor vehicle accident at the corner of Byrne Road and Arlington Avenue on Sunday, July 19, 2026. Crews found one vehicle on its side with a patient still inside. The vehicle was stabilized and the patient was extricated through the windshield. One patient was transported to an area hospital for evaluation of injuries.

TFRD responded to a residential structure fire with reports of one person possibly trapped inside on Hollywood Avenue near Bancroft Street on July 15, 2026.

Neighbors reported hearing an explosion before seeing smoke and flames coming from the home. They also reported the occupant was likely still inside.

Fire crews arrived to find heavy fire and smoke coming from the structure. They made an aggressive interior attack while simultaneously conducting a primary search. During the search, firefighters located the victim inside the home and removed them to a waiting ambulance, where they received medical treatment. The patient was transported to a local hospital in critical condition and was later pronounced dead.

While firefighters were still inside the structure, multiple explosions were heard from within the home. Intense heat and hoarding conditions forced crews to withdraw. A second alarm was requested, bringing additional engine companies and medic units to the scene for additional manpower. Firefighters later re-entered the structure with fresh crews and continued interior operations, but deteriorating fire conditions forced a second evacuation. Incident commanders then transitioned to a defensive strategy.

Truck 17’s elevated master stream and multiple handlines were used to extinguish the fire and protect surrounding structures. No neighboring homes sustained significant damage.

Two firefighters were transported to a local hospital in stable condition. The Toledo Fire Investigation Unit is investigating the cause of the fire.

Three people have died and two children remain hospitalized in critical condition following a tragic incident on July 15, 2026, that investigators believe was caused by accidental carbon monoxide poisoning.

Toledo Fire & Rescue crews were initially dispatched for a report of a person slumped over in a vehicle. Upon arrival, responders found five unconscious patients, with law enforcement already performing CPR. A mass casualty incident was declared, and due to the unknown cause of the patients’ condition, a hazardous materials response was requested.

All five patients were transported to area hospitals in critical condition. Hazardous materials technicians later detected high levels of carbon monoxide inside the running vehicle.

Mayor Wade Kapszukiewicz said the incident began after a family stopped along the roadside because of a flat tire. While stopped, investigators believe carbon monoxide accumulated inside the running vehicle, resulting in the deaths of three family members and leaving two children in critical condition.

Authorities do not currently suspect foul play, though the investigation remains ongoing.
